Bear Lake Memorial Hospital’s Medical Records is open Monday through Friday from 8:30 a.m. to 5 p.m. If you would like a copy of your medical records, you must complete an Authorization for Release of Medical Records and forward it to us in person, by mail or via fax. The mailing address is: Our phone number is (208) 847-0932 This form must be completely filled out, signed and dated by the patient or legal representative.
